Virtual Healthcare Appointment Instructions:     

Before your appointment please do the following:

1. Register on our Patient Portal at threeriverspublichealth.org or click HERE. The email you provided when scheduling the visit will be your username. 
2. Complete and submit the Medical History questionnaire on the Patient Portal. 
3. Download the Healow App to access your account. This is OPTIONAL and FREE. The practice code for Three Rivers is: EEIECA                                             

Starting your Virtual Healthcare visit:

There are several ways you can start your visit:

1. Log in on the Patient Portal with your username and password and start the visit. 
2. Click on the link from either your email or text reminder. No log in is needed with this option.
3. Start the visit through the Healow app. 

After your Visit:

• Your healthcare provider will schedule a follow-up, in-person appointment if needed.
• You will receive a short survey via email or text. You can also access the survey by clicking HERE We encourage you to complete the survey so we can improve our services!
